Abstract

Los avances ocurridos en los últimos años en medicina, en especial de los cuidados intensivos postoperatorios, han determinado un aumento importante en la supervivencia de niños que hasta hace poco tiempo morían de forma irremediable. Sin embargo es posible sostener la vida más allá del punto en que la supervivencia no es factible y sólo se logra aumentar el sufrimiento del paciente. Las decisiones de limitación del esfuerzo terapéutico y cuando considerar la futilidad de un tratamiento, son polémicas y difíciles de tomar, generando gran estrés intelectual y emocional en las personas que tienen la responsabilidad de decidir, sobre todo en pediatría. El presente trabajo tiene como objetivo reflexionar sobre dilemas éticos que se nos presentan en el postoperatorio de la cirugía cardiovascular pediátrica, basados en los principios de la bioética y del conocimiento más actual según la medicina basada en la evidencia, para fomentar discusiones y elaborar recomendaciones concretas que nos ayuden a la toma de decisiones clínicas.
